Please send the following four (4) documents to get started:

1. Application (see below);
2. Signed Fee Agreement (see below);
3. Court Order/Settlement Agreement that specifies how the retirement account is to be divided; and
4. A recent account statement.

Once we have these documents, we’ll email a payment link. Upon payment, we’ll promptly draft the order and submit it for pre-approval.

Our work includes:

• Drafting and submitting the initial order to Plan Administrator for pre-approval;
• Revising the order (if needed); and
• Submitting the pre-approved order to your attorney for filing and final submission.
  (If you will not be using an attorney, please use the “with filing” Fee Agreement below, and we’ll facilitate the filing and submission of your order.)

Simply QDROs will assist in any way needed to ensure final approval.
